【目的】 观察循经远取腧穴针刺治疗原发性三叉神经痛的临床疗效。【方法】 选取 2023年 2月至 2024年 1月复旦大学附 属中山医院青浦分院收治的60例明确诊断为原发性三叉神经痛的患者为研究对象。按照随机信封法将患者随机分为观察组 和对照组，每组各 30例。对照组给予卡马西平治疗，观察组在对照组治疗的基础上，给予循经远取腧穴针刺治疗。连续治 疗4周。治疗4周后，评价2组临床疗效。观察2组患者治疗前后疼痛视觉模拟量表（VAS）评分的变化情况，以及患者每次疼 痛发作的持续时间。比较 2 组患者治疗前后简明生活质量量表（SF-36）评分的变化情况。【结果】（1）观察组总有效率为 96.67%（29/30），对照组为 76.67%（23/30）。观察组疗效优于对照组，差异有统计学意义（P＜0.05）。（2）治疗后，2组患者的 VAS评分明显改善（P＜0.05），且观察组在改善VAS评分方面明显优于对照组，差异有统计学意义（P＜0.05）。（3）治疗后，2组 患者的疼痛发作持续时间明显改善（P＜0.05），且观察组在改善疼痛发作持续时间方面明显优于对照组，差异有统计学意义 （P＜0.05）。（4）治疗后，2组患者的 SF-36生活质量评分明显改善（P＜0.05），且观察组在改善 SF-36生活质量评分方面明显 优于对照组，差异有统计学意义（P＜0.05）。【结论】 循经远取腧穴针刺联合卡马西平治疗原发性三叉神经痛，能明显改善患 者的疼痛症状，减少患者疼痛的持续时间，从而提高患者的生活质量，疗效显著。

Objective To observe the clinical efficacy of distal acupoint selection along meridians acupuncture in the treatment of primary trigeminal neuralgia. Methods Sixty patients with a confirmed diagnosis of primary trigeminal neuralgia， admitted to Qingpu Branch of Zhongshan Hospital Affiliated to Fudan University between February 2023 and January 2024，were selected as the study subjects. A random envelope method was applied to randomly assigned the patients to an observation group and a control group， with 30 cases in each group. The control group received carbamazepine treatment， while the observation group received distal acupoint selection along meridians acupuncture in addition to the same carbamazepine treatment as the control group. The treatment course lasted for 4 consecutive weeks. Four weeks after treatment， the clinical efficacy was evaluated for both groups. Changes in the Visual Analogue Scale （VAS） scores and the duration of each pain episode were observed before and after treatment in both groups. Changes in the scores of the Short Form-36（SF-36） quality of life scale were also compared between the two groups before and after treatment. Results （1） The total effective rate was 96.67% （29/30） in the observation group and 76.67% （23/30） in the control group. The efficacy of the observation group was superior to that of the control group，and the difference was statistically significant （P＜0.05）.（2） After treatment，the VAS scores were significantly improved in both groups （P＜0.05），and the improvement in VAS scores was significantly greater in the observation group than the control group， with a statistically significant difference （P＜0.05）.（3） After treatment，the duration of pain episodes was significantly improved in both groups （P＜0.05），and the improvement was significantly greater in the observation group than the control group，with a statistically significant difference （P＜0.05）. （4） After treatment， the SF-36 quality of life scores was significantly improved in both groups （P＜0.05），and the improvement was significantly greater in the observation group than the control group，with a statistically significant difference （P＜0.05）. Conclusion The combination of distal acupoint selection along meridians acupuncture and carbamazepine in treating primary trigeminal neuralgia can significantly alleviate patients’ pain symptoms，reduce the duration of pain episodes，and thereby improve patients’ quality of life，demonstrating significant therapeutic efficacy.
