【目的】 探讨补肾促排卵汤对多囊卵巢综合征(PCOS)大鼠的治疗作用及机制。【方法】 将SD大鼠随机分为正常组、模 型组、补肾促排卵汤组、补肾促排卵汤+EX-527(SIRT1抑制剂)组,每组9只。除正常组,其余组大鼠通过颈部皮下注射硫 酸脱氢表雄酮构建PCOS模型。分组干预后,检测各组大鼠的动情周期,苏木精-伊红(HE)染色法观察卵巢组织中卵泡的形 态学变化,放射免疫法检测血清中雌二醇(E2 )、卵泡刺激素(FSH)、睾酮(T)、促黄体生成素(LH)水平,16s rRNA测序法对 肠道菌群进行测序分析,微量法检测卵巢组织中超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、丙二醛(MDA)水 平,实时定量聚合酶链反应(RT-qPCR)法检测卵巢组织中 SIRT1、PGC-1α 的 mRNA 表达,Western Blot 法检测卵巢组织中 SIRT1、PGC-1α的蛋白表达。【结果】 与正常组比较,模型组大鼠动情周期紊乱,卵巢组织中黄体数量减少,可见多个囊性 扩张卵泡,血清中 T、LH 水平升高(P<0.05),血清中 E2、FSH 水平降低(P<0.05),肠道菌群 α 多样性指标 Chao1、 Shannon、Simpson 降低(P<0.05),肠道菌群厚壁菌门(Firmicutes)、普雷沃菌属(Prevotella)相对丰度升高,拟杆菌门 (Bacteroidetes)、软壁菌门(Tenericutes)、乳酸杆菌(Lactobacillu)相对丰度降低,卵巢组织中MDA水平升高(P<0.05),卵巢组 织中SOD、GSH-Px水平降低(P<0.05),卵巢组织中SIRT1、PGC-1α 的mRNA及蛋白表达水平降低(P<0.05);与模型组比 较,补肾促排卵汤组大鼠动情周期紊乱减轻,卵巢组织中黄体数量增加,囊性扩张卵泡减少,血清中T、LH水平降低(P< 0.05),血清中 E2、FSH水平升高(P<0.05),肠道菌群 α多样性指标 Chao1、Shannon、Simpson升高(P<0.05),肠道菌群厚 壁菌门、普雷沃菌属相对丰度降低,拟杆菌门、软壁菌门、乳酸杆菌相对丰度升高,卵巢组织中MDA水平降低(P<0.05), 卵巢组织中 SOD、GSH-Px 水平升高(P<0.05),卵巢组织中 SIRT1、PGC-1α 的 mRNA 及蛋白表达升高(P<0.05);EX-527 能抑制补肾促排卵汤的改善作用(P<0.05)。【结论】 补肾促排卵汤能减轻PCOS大鼠的肠道菌群紊乱及氧化应激损伤,其机 制可能与激活SIRT1/PGC-1α信号通路相关。

[Abstract]
Objective To investigate the therapeutic effect and mechanism of Bushen Cupailuan Decoction (BCD) in rats with polycystic ovary syndrome (PCOS). Methods SD rats were randomly divided into a normal group, a model group, a BCD group, and a BCD + EX-527(a SIRT1 inhibitor) group, with 9 rats in each group. Except the normel group,a PCOS rat model was established in in the other groups by subcutaneous injection of dehydroepiandrosterone sulfate into the neck. After grouping intervention,the estrous cycle was monitored in each group. Morphological changes of follicles in ovarian tissue were observed by HE staining. Serum levels of estradiol (E2 ),follicle-stimulating hormone (FSH),testosterone (T),and luteinizing hormone (LH) were measured by radioimmunoassay. Gut microbiota was sequenced and analyzed via 16S rRNA sequencing. Levels of superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), and malondialdehyde(MDA) in ovarian tissue were detected by micro-methods. The mRNA expression of SIRT1 and PGC-1α in ovarian tissue was determined by RTqPCR,and their protein expression was measured by Western Blot. Results Compared with the normal group,the model group exhibited disordered estrous cycles, a decreased number of corpora lutea, and the presence of multiple cystic dilated follicles in the ovarian tissue,increased serum T and LH levels (P<0.05),decreased serum E2 and FSH levels (P<0.05),reduced α diversity indices of gut microbiota (Chao1,Shannon,Simpson, P<0.05), an increased relative abundance of Firmicutes and Prevotella in the gut microbiota, alongside decreased relative abundances of Bacteroidetes,Tenericutes,and Lactobacillus,elevated MDA level (P<0.05) and decreased SOD and GSH-Px levels (P<0.05) in ovarian tissue, and downregulated mRNA and protein expression of SIRT1 and PGC-1α in ovarian tissue (P<0.05). Compared with the model group,the BCD group showed alleviated estrous cycle disorder, an increased number of corpora lutea, and reduced cystic dilated follicles in the ovarian tissue,decreased serum T and LH levels (P<0.05),increased serum E2 and FSH levels (P<0.05),elevated α diversity indices of gut microbiota (Chao1,Shannon,Simpson,P<0.05),a decreased relative abundance of Firmicutes and Prevotella, alongside increased relative abundances of Bacteroidetes, Tenericutes,and Lactobacillus in the gut microbiota,reduced MDA level (P<0.05) and increased SOD and GSHPx levels (P<0.05) in ovarian tissue,and upregulated mRNA and protein expression of SIRT1 and PGC-1α in ovarian tissue (P<0.05). EX-527 inhibited the improvement effects of BCD (P<0.05). Conclusion Bushen Cupailuan Decoction can ameliorate gut microbiota dysbiosis and oxidative stress injury in PCOS rats, and its mechanism may be associated with the activation of the SIRT1/PGC-1α signaling pathway.
