急性踝关节扭伤作为临床常见的运动系统损伤，症状骤现，恢复缓慢，严重影响患者的运动功能与生活质量。李飞 教授认为，外力因素致经筋挛结、气血壅滞为本病核心病机，病机总体呈现“筋结阻络-宿瘀留滞-虚中夹滞”三阶演变， 主张以通痹复衡、动络调衡、固本续衡之法分期论治，并提出“一期松解、二期通调、三期固本”，即“一刀二针三巩固” 的分期治疗策略：首以针刀松解筋结，破除粘连；次以动气针法联合温针灸法畅调气血，荡涤瘀浊；再以中药熏渍温养相 合，巩固疗效。融合中医外治精髓，构建“破中有补、动中寓静”的整合干预模式，以期为急性踝关节扭伤的临床诊疗提 供新思路。

Acute ankle sprain， a common clinical injury of the musculoskeletal system， is characterized by sudden symptom onset and slow recovery， significantly impairing patients’ motor function and quality of life. Professor Li Fei posits that the core pathogenesis of this condition involves contraction of the sinews and stagnation of qi and blood due to external trauma. The pathological mechanism generally evolves through three stages：“sinew node-induced collateral obstruction”，“lingering stasis retention”，and “deficiency complicated by stagnation”. Consequently，he advocates for staged treatment aimed at unblocking obstruction and restoring balance，activating collaterals and regulating equilibrium，and reinforcing the root and sustaining stability. He has proposed a staged therapeutic strategy of “release in the initial stage， regulation and unblocking in the middle stage， and consolidation of the root in the final stage”. This approach entails initially using acupotomy to release sinew nodes and break down adhesions；subsequently applying moving qi acupuncture combined with warm needling to regulate qi and blood and dispel stasis and turbidity； and finally employing herbal fumigation and immersion to warmly nourish and consolidate therapeutic effects. By integrating the essence of external treatments in traditional Chinese medicine，this model constructs a comprehensive intervention strategy embodying the principles of “supplementation within dispersion” and “tranquility within activity”， offering a novel perspective for the clinical diagnosis and treatment of acute ankle sprain.
