【目的】 基于R语言数据挖掘技术分析火针治疗膝骨关节炎的选穴规律,为临床应用提供选穴依据。【方法】 通过计算 机检索中国知网期刊全文数据库(CNKI)、万方学术期刊全文数据库(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物 医学数据库(CBM)及美国生物医学信息检索系统(PubMed)、美国科学网(Web of Science)等各大数据库从 2005 年 1 月 1 日 至 2025 年 4 月 30日收录的有关火针治疗膝骨关节炎的临床研究类文献,利用 Excel 2021建立腧穴处方数据库,采用 R语言 对腧穴使用频次、归经、特定穴情况进行统计,并进行相关性、关联规则分析和聚类分析。【结果】 共纳入121篇文献,包含 31个腧穴,总频次为725次。高频腧穴为内膝眼、犊鼻、阳陵泉等,常用经脉为足阳明胃经、足太阴脾经、足少阳胆经,五 输穴-合穴为使用频次最高的特定穴。相关性分析表明:命门、太冲、申脉,膝关、委阳、承筋,内膝眼、犊鼻三组腧穴之 间的配伍密切。关联规则分析显示:内膝眼-犊鼻为支持度最高的穴对,梁丘,阳陵泉-血海的提升度最高,将腧穴分为 3 大类聚类。【结论】 火针治疗膝骨关节炎的选穴规律是以疏通经络、滋补肝肾为原则,结合局部取穴,核心腧穴为犊鼻、内 膝眼、阳陵泉、血海、阿是穴、梁丘、足三里。

[Abstract]
Objective To analyze the acupoint selection patterns in fire needle treatment for knee osteoarthritis (KOA) using R language data mining technology, and to provide a reference for clinical acupoint selection. Methods Clinical research literature on fire needle treatment for KOA published between January 1,2005,and April 30,2025,was retrieved through computerized searches of databases including China National Knowledge Infrastructure (CNKI),Wanfang Academic Journal Full-text Database,VIP Chinese Sci-Tech Journal Database, China Biology Medicine disc (CBM), PubMed, and Web of Science. An acupoint prescription database was established using Excel 2021. R language was employed to perform statistical analyses on acupoint usage frequency, meridian tropism, and specific acupoint categories, along with correlation analysis, association rule analysis, and cluster analysis. Results A total of 121 articles were included,involving 31 acupoints with a total frequency of 725. High-frequency acupoints included Neixiyan(EX-LE4),Dubi(ST35),Yanglingquan(GB34),among others. The most commonly used meridians were the stomach meridian of foot-yangming,the spleen meridian of foot-taiyin, and the gallbladder meridian of foot-shaoyang. Among the five-shu points-he-sea points were the most frequently used specific acupoint category. Correlation analysis indicated closecompatibility relationships between three groups of acupoints:Mingmen(GV4),Taichong(LR3),and Shenmai(BL62);Xiguan(LR7), Weiyang(BL39),Chengjin(BL56);Neixiyan and Dubi. Association rule analysis revealed that the acupoint pair Neixiyan(EX-LE4)-Dubi(ST35) had the highest support,while the combination Liangqiu(ST34),YanglingquanXuehai(SP10) had the highest lift. Cluster analysis grouped the acupoints into three major categories. Conclusion The acupoint selection pattern for fire needle treatment of knee osteoarthritis is based on the principles of dredging the meridians and tonifying the liver and kidney,combined with local point selection. The core acupoints are Dubi, Neixiyan,Yanglingquan,Xuehai,Ashi points,Liangqiu,and Zusanli (ST36).
