脑缺血再灌注损伤(CIRI)是脑血管疾病中的关键病理过程,缺血后血流恢复会引发一系列复杂的神经生理与生化改 变,其中神经细胞凋亡是导致中风后神经功能严重损害的核心环节。CIRI中神经细胞凋亡的病理生理机制涉及多个层面。 针灸作为传统中医疗法,在干预神经细胞凋亡方面展现出独特的作用机制。尽管针灸在干预CIRI神经细胞凋亡方面已取得 一定研究成果,但目前其具体作用机制仍未完全明晰,且临床研究的深度和广度有待进一步提升。该文对近年来相关研究 进行系统总结,分析针灸干预神经细胞凋亡的作用机制及临床研究进展,并对未来研究方向提出展望,以期为脑卒中等脑 血管疾病的治疗提供更为坚实的理论依据与有效的临床指导。

Cerebral ischemia-reperfusion injury (CIRI) represents a critical pathological process in cerebrovascular diseases. The restoration of blood flow following ischemia triggers a series of complex neurophysiological and biochemical changes, among which neuronal apoptosis is a central mechanism leading to severe neurological impairment after stroke. The pathophysiological mechanisms of neuronal apoptosis in CIRI involve multiple levels. Acupuncture and moxibustion,as traditional Chinese therapeutic modalities,demonstrate unique mechanisms of action in intervening in neuronal apoptosis. Although certain research achievements have been made regarding the intervention of acupuncture and moxibustion in neuronal apoptosis following CIRI, the specific mechanisms of action remain incompletely elucidated, and the depth and breadth of clinical research require further enhancement. This article systematically summarizes recent relevant studies,analyzes the mechanisms and clinical research progress of acupuncture and moxibustion in intervening in neuronal apoptosis,and proposes perspectives for future research directions,aiming to provide a more solid theoretical foundation and effective clinical guidance for the treatment of cerebrovascular diseases such as stroke.
