[摘要]
【目的】 观察电针阳陵泉和曲池穴对大鼠脑卒中肢体痉挛的治疗作用及机制。【方法】 将大鼠随机分为假手术组、模型 组、电针组和电针+CA1[Janus激酶 2(JAK2)激动剂]组。除假手术组,其余各组大鼠采用改良的大脑中动脉线栓联合内囊注 射N-甲基-D-天冬氨酸(NMDA)受体法制备脑卒中肢体痉挛模型。干预后,对大鼠进行神经功能缺损评分、阿什沃斯肌肉痉 挛评定量表(Ashworth)评分和电生理测定;酶联免疫吸附分析(ELISA)检测脑组织中白细胞介素 6(IL-6)、肿瘤坏死因子 α (TNF-α)、γ-氨基丁酸(GABA)水平;采用比色法检测谷氨酸(Glu)含量;苏木素-伊红(HE)染色法观察脑组织病理学变 化;Western Blot 法检测脑组织中 JAK2、磷酸化 JAK2(p-JAK2)、信号转导子和转录激活子 3(STAT3)、磷酸化 STAT3 (p-STAT3)蛋白表达。【结果】 模型组神经功能缺损评分,Ashworth评分,脑组织中IL-6、TNF-α、Glu含量及p-JAK2/JAK2、 p-STAT3/STAT3 比值均显著高于假手术组(P < 0.05),肌张力信号值、脑组织中 GABA 含量显著低于假手术组(P < 0.05); 与模型组比较,电针组神经功能缺损评分,Ashworth评分,脑组织中IL-6、TNF-α、Glu含量及p-JAK2/JAK2、p-STAT3/STAT3 比值均显著降低(P < 0.05),肌张力信号值、脑组织中 GABA 含量显著升高(P < 0.05);与电针组比较,电针+CA1 组神经 功能缺损评分,Ashworth评分,脑组织中IL-6、TNF-α、Glu含量及p-JAK2/JAK2、p-STAT3/STAT3比值显著升高(P < 0.05), 肌张力信号值、脑组织中GABA含量显著降低(P < 0.05)。【结论】 电针可能通过抑制JAK2/STAT3通路抑制炎症反应,调节神 经元兴奋/抑制平衡,从而缓解脑卒中大鼠肢体痉挛。

[Abstract]
Objective To observe the therapeutic effect and mechanism of electroacupuncture at Yanglingquan (GB34)and Quchi(LI11)points for post-stroke spasticity(PSS)in rats. Methods The rats were randomly divided into sham-operated group,model group,electroacupuncture group and electroacupuncture + CA1 [Janus kinase 2 (JAK2)agonist] group. The PSS model was prepared by the modified middle cerebral artery occlusion combined with internal capsule injection of N-methyl-D-aspartate(NMDA)receptor method in all groups of rats except the sham-operated group. After the intervention, the neurological deficit scale scoring, Ashworth Spasticity Scale (Ashworth)scoring and electrophysiological determination were carried out;the levels of interleukin 6(IL-6),tumor necrosis factor α(TNF-α)and γ-aminobutyric acid(GABA)in the brain tissues were detected by enzymelinked immunosorbent assay(ELISA);the content of glutamate(Glu)was observed by colorimetry;the pathological changes in brain tissues were observed by hematoxylin-eosin(HE)staining method;the protein expression levels of JAK2, phosphorylated JAK2(p-JAK2), signal transducer and activator of transcription 3(STAT3), and phosphorylated STAT3 (p-STAT3) in brain tissues were detected by Western Blot method. Results The neurological deficit scale scores,Ashworth scores,contents of IL-6,TNF-α and Glu as well as p-JAK2/JAK2 and p-STAT3/STAT3 ratios in brain tissues in the model group were all significantly higher than those in the shamoperated group(P < 0.05),and the muscular tension signal value and GABA content in brain tissues in the model group were significantly lower than those in the sham-operated group(P < 0.05);compared with the model group, the neurological deficit scale scores,Ashworth scores,contents of IL-6,TNF-α and Glu as well as p-JAK2/ JAK2 and p-STAT3/STAT3 ratios in brain tissues were significantly lower in the electroacupuncture group(P < 0.05),and the muscular tension signal value and GABA content in brain tissues were significantly higher in the electroacupuncture group(P < 0.05);compared with the electroacupuncture group,the neurological deficit scale scores,Ashworth scores,contents of IL-6,TNF - α and Glu as well as p-JAK2 / JAK2 and p-STAT3 / STAT3 ratios in brain tissues in the electroacupuncture + CA1 group were significantly higher(P < 0.05), and the muscular tension signal value and GABA content in brain tissues in the electroacupuncture + CA1 group were significantly lower(P < 0.05). Conclusion Electroacupuncture may inhibit inflammatory responses and regulate the neuronal excitation/inhibition balance by inhibiting the JAK2/STAT3 pathway,thus relieving PSS in rats.
